Handicap Placard (hangs on rearview mirror)
Types of placard applications are: permanent, temporary, severely disabled veteran placard, renewal, replacement, change of name and/or address. Permanent placards need renewed every 5 years. Temporary placards are good for no more than 6 months.
Applicant Eligibility Requirements for a placard include:
Applicant is:
- Blind
- Does not have full use of an arm or both legs
- Cannot walk 200 ft without stopping to rest
- Cannot walk without the use of, or assistance from, a brace, cane, crutch, another person, prosthetic devise, wheelchair or other assistive device
- Is restricted by lung disease to such an extent that the person's forced (respiratory) expiratory volume for one second, when measured by spirometry, is less than one liter or the arterial oxygen tensions is less than 60 MM/HG or room air at rest
- Uses portable oxygen
- Has a cardiac condition to the extent that the person's functional limitations are classifieds in severity as Class III or Class IV according to the standards set by the America Heart Association
- Is severely limited in his or her ability to walk due to an arthritic, neurological or orthopedic condition
- Is a person in loco parentis of a person specified above
We have forms available in our office for your use. Your doctor may also have these forms available. After having your doctor complete the form, please bring in to our office with your valid PA drivers license or Photo ID and we can notarize the form and send using our messenger service. The placard will be sent back to our office within 7-10 days.
Disability or Hearing Impaired Registration Plate or Person with a Disability Motorcycle Plate
Eligibility Requirements are the same as those for a placard application. (see above)
For a hearing impaired plate, any person with a hearing impairment verified by a licensed health care provider is eligible.
